MONET SALON HAIR EXTENSIONS MEMBERSHIP
First: Client hereby authorizes the Salon to bill Client's credit card or debit card a onetime enrollment fee today in the amount of the Enrollment Fee.
Second: Client hereby authorizes the Salon to bill Client's credit card or debit card the Monthly Fee for twelve 30-day cycles on or about the same day of each 30-day cycle, with the first of the twelve-Monthly Fees being billed today.
Third: As a member of the Hair Extensions Membership Program, the Client may schedule five "Installations Membership Appointments" per year (365 days) at the Salon for "Installations Services," and the Salon shall perform such Installation Services at no additional charge.
Fourth: As a member of the Hair Extensions membership Program, the client will receive two Sets of New JC Bella Hair Extensions.
Fifth: Hair Extensions Installation Services are limited to Removal and Installation of the same hair given by the salon.
Sixth: Client acknowledges that services other than those specifically identified as Membership Services are not included as part of the Hair Extensions Membership Program and are subject to the Salon's customary fees. For example, services, such as Hair Coloring, Highlights, Lowlights, Ombre, Balayage, Haircut, and Blow Outs are not included. Also smoothing treatment (e.g., Keratin, Brazilian Blowout, etc.), and additives (e.g., boosters, bonders, protectors, Olaplex etc.), are not Hair Extensions Membership Services and will be billed to Client and paid at time of service.
Seventh: Client acknowledges and agrees that Monet Salon may from time to time require a change of a Client's "Membership" appointment and/or stylist, (at The Salon discretion) to best fulfill its obligation to Client under this agreement.
Eighth: Client acknowledges that each 365-day cycle allows for five Installation Membership Appointments; and two new sets of Hair, Membership Appointments and/or sets of new Hair are not accrue or roll-over to subsequent 365-day cycles.
Ninth: Membership Appointments and new sets are for the Client only; benefits of the Hair Extensions Membership Program are not assignable or transferable.
Tenth: Client acknowledges and agrees to pay a fee of $40.00 for any Monthly Fee declined and/or reversed.
Eleventh: Client acknowledges and agrees that she/he is required to provide at least 24 hours' notice of cancellation for any scheduled Membership Appointment.
Twelfth: Client agrees, our stylist to use reasonable judgment in determining their hair service needs.
Thirteenth: Client acknowledges that tips to stylists are not included in the membership fees.
Fourteenth: Client acknowledges and agrees that Client's preferred scheduling for Membership Appointments and Client's request for a certain stylist is subject to availability.
Fifteenth: The Salon may terminate this contract at any time with or without cause.
Sixteenth: This contract is intended to continue the Client's membership after the twelve 30-day cycles on a month-to-month basis until cancelled.
Seventeenth: The Salon won't reimburse the client for any unused membership Appointments for the 30-day cycle in which cancellation of the contract occurs.
Eighteenth: Client hereby authorizes the Salon to bill Client's credit card or debit card to continue the Client's participation in the Hair Extensions Membership Program on a month-to-month basis.
